    STUDENT/PARENT AGREEMENT

    Academic Eligibility: 

    In order to qualify for placement on any team, a student must have earned a minimum of a 2.0 grade point average (GPA) with no more than one F on the previous grade report.  A student must also maintain a satisfactory discipline record (see below).

    A student who is passing five classes, but whose GPA is between a 1.75 & 1.99 may request to be placed on “Academic Probation” for a three-week period.  During such probation, the student may practice with the team but may not participate in any contests.  After the three weeks, & the successful attainment of a 2.0 GPA, the student’s full participation is granted.  *Academic probation will only be granted once to any student for the entire enrollment at KJHS.

    Attendance & Conduct Eligibility:

    1. A student athlete must attend school ­­ALL DAY in order to practice or play.  An occasional exception may be made if cleared by an administrator with advance notice of the absence.

    2. Conduct on and off the Kenilworth campus will be of a nature to bring credit and pride to our junior high and our teams.  A student athlete that commits any offense that results in a suspension from school or an In-House Suspension (IHS) will not participate in practice or competition for the duration of the suspension or on the day of IHS.  Students suspended for the use, intent to distribute/sell or possession of any illegal substance, including alcohol and tobacco, will be expelled from the team.  The student will also forfeit all awards and honors.  Any violations of the law outside of school will be handled on a case-by-case basis by the athletic department. 

    3. Student athletes will not practice or play that day if not participating in P.E. for any reason.

    4. Regular and punctual attendance to all practices and contests is required unless prior excuse is obtained from the coach.  Quitting a team is prohibited.

    5. The student athlete is expected to conduct themselves in an appropriate and respectful manner at all times.  They will treat all other teams, coaches, referees, & officials courteously.  Any offense of verbal or physical altercations during a contest will result in disciplinary action.  The student athlete will use all equipment appropriately & with respect.  They will wear and treat their uniforms with respect.  Student’s may be billed for any damage or loss incurred.

    Sudden Cardiac Arrest (SCA) in Student-Athletes

    What Is Sudden Cardiac Arrest? - Sudden Cardiac Arrest (SCA) is not a heart attack. It’s an abnormality in the heart’s electrical system that abruptly stops the heartbeat. It's caused by an undetected congenital or genetic heart condition.

    First Symptom May Be Death -Possible Warning Signs: Unexplained fainting, shortness of breath, lightheadedness, chest pain, heart palpitations, family history of SCA or unexplained sudden death under the age of 50.

    1 Youth Athlete Dies Every 3 Days in the United States - SCA is 60% more likely to occur during exercise or sports activity, so athletes are at greater risk. SCA is also the leading cause of death on school campuses.

    Time Critical - If not properly treated within minutes, SCA is fatal in 92% of cases.

    Are there warning signs? - Although SCA happens unexpectedly, some people may have signs or symptoms, such as:

    • dizziness
    • lightheadedness
    • shortness of breath
    • difficulty breathing
    • racing or fluttering heartbeat (palpitations)
    • syncope (fainting)
    • fatigue (extreme tiredness)
    • weakness
    • nausea
    • chest pains

    These symptoms can be unclear and confusing in athletes. Often, people confuse these warning signs with physical exhaustion. SCA can be prevented if the underlying causes can be diagnosed and treated.

    Voluntary Activities Acknowledgement and Assumption of Risk. 

    The below-named student wishes to participate in the District-sponsored athletic program. We understand and acknowledge that these activities by their very nature, pose the potential risk of serious injury/illness to individuals who participate in such activities.

    • We understand and acknowledge that some of the injuries/illnesses which may result from participating in these activities include but are not limited to the following: sprains/strains, fractured bones, unconsciousness, head and/or back injury, paralysis, loss of eyesight, communicable diseases, and death.
    • We understand and acknowledge that participation in these activities is completely voluntary and as such is not required by the School.
    • We understand and acknowledge that in order to participate in these activities the parent/legal guardian agrees to assume liability and responsibility for any and all potential risks that may be associated with participation in such activities.
    • We understand, acknowledge and agree that the School, its employees, officers, agents, or volunteers shall not be liable for any injury/illness suffered by the student athlete which is incident to and/or in association with preparing for and/or participating in this activity.

    Student Agreement:
    • Show respect for teammates, coaches, opponents, officials, and myself.
    • Use no foul language, trash talk, negative gestures, or actions to provoke a negative response or fighting.
    • Be in attendance at school a minimum of two (2) periods each day before I will be allowed to practice or play in a contest.
    • Maintain a minimum of 2.0 GPA and pass four (4) classes according to the policies of the Petaluma City School District and the
    Governing Board of the North Coast Section.

    Parent/Legal Guardian Agreement:
    • The above named student resides in the Petaluma City School District, or has an approved inter-district transfer and will abide by the
    policies of the Petaluma City School District for residency.
    • I permit the above named student to compete in interscholastic athletics and travel to away competitions. If the above named student is
    injured, the coach and/or school official is authorized to have him/her treated.
